Cottage View Health Center
In December 2002, NNHSC opened its fourth primary care site in Chicago. Located on the first floor of the Cottage View Terrace senior residential building, Cottage View Health Center's primary focus is serving the health needs of the senior population of the Grand Boulevard, Kenwood, and Oakland communities on the South side of Chicago. This new site provides a significantly more accessible source of care not only for the seniors residing in the building housing the new health center, but also for those seniors residing in 12 nearby senior housing sites. While we anticipate primarily targeting our services to the senior population, this site will also be available to provide clinical evaluation, primary care and referral for persons of all ages residing in the community.
Our providers place special emphasis on treatment and prevention of such chronic diseases as diabetes, coronary heart disease, emphysema, cancer, and arthritis. In addition to primary care, NNHSC focuses special attention on providing health education, ensuring medication adherence, monitoring medication interactions, facilitating activities of daily living, and providing group classes, lectures, social interactions, and linkage activities with Komed Holman Health Center and nearby community based organizations which serve seniors.

Near North Health Service Corporation offers a full range of primary clinical services including complete lab and x-ray capability. Providers offer obstetrical and gynecological care for women, which includes preventative measures such as regular mammogram screenings and family planning. Pediatricians offer comprehensive health care to infants, children, and adolescents. Physicians place emphasis on the importance of immunization and the prevention of chronic diseases. Special attention is given to diabetic eye care and NNHSC provides general eye care including treatment for cataracts and glaucoma. Similarly, our podiatrist places emphasis on foot care for those suffering from diabetes.
Internal Medicine (Adult Medicine)
Our Internal Medicine department provides comprehensive primary care and health maintenance for adults. We provide complete medical and behavioral assesments for geriatric patients, nutritional counseling for patients with diabetes, hypertension, hyperlipidemia, and obesity, comprehensive HIV care, family planning, STD diagnosis and treatment, and hormone replacement therapy. Our providers are a part of the Diabetes Collaborative, a nationwide study designed to assess various interventions on likelihood for development of diabetes in at risk minority populations. We also offer smoking cessation classes.
Women's Health (Obstetrics & Gynecology)
Our Women's Health department provides prenatal care, family planning, STD diagnosis and treatment, comprehensive evaluation and diagnosis of gynecologic conditions, hormone replacement therapy, and evalution and management of abnormal pap smears.
Pediatrics (Children's Doctor)
Our Pediatrics department provides complete primary care to children from ages 0 to 17, adolescent medicine including teen physicals and family counseling, primary care for the chronically ill child including children with multiple handicaps, lead education and immunizations. Several of our providers are members of the Pediatric Practice Research Group, which conducts carefully selected research projects to promote our understanding of medical issues affecting our patients and to develop better interventions.
Podiatry (Foot Doctor)
Our Podiatrists provide evaluation and treatment of problems of the foot and ankle especially those resulting from diabetes and vascular disease. Treatment of common podiatric problems such as bunions and hammer toes are also provided. Our providers also provide assessments of congenital problems and gait disturbances.
Ophthalomology (Eye Doctor)
Our Ophthalmologist provides evaluation and treatment of diseases of the eye, especially those resulting from diabetes, hypertension, glacoma, cataracts and vascular disease.
Oral Health (Dentist)
Near North Health Service Corporation provides much needed dental services to our patient population. NNHSC's Oral Health Department provides general dentistry: x-rays, cleanings, fillings, extractions, and sealants.
Mental Health
NNHSC provides mental health services to patients who are suffering from both acute and chronic psychological/emotional conditions. We offer both medication management and supportive talk therapy to treat conditions such as anxiety, depression, stress, and emotional distress.

The Near North Comprehensive Services staff provides support services to children, families, and seniors. These services include Family Case Management, Family Support Services, Social Work Services, Substance Abuse Treatment and the Youth Initiative. Social service and medical based case management support are offered to pregnant and parenting mothers and their children. First-time parents are provided with home visits and training in parent-child development. Family support services also provides support for grandparents responsible for child rearing. The department has developed intergenerational programs to strengthen families and the communities in which they live. In addition, we provide outpatient substance abuse treatment, which employs a variety of proven techniques including acupuncture.
The Division of Medical Social Work
Professional social work support is available to patients using health services at any of our health centers. This support may be in the form of case management (assess and link: assessing the patient need and linking to appropriate community services), individual counseling with a state licensed social worker, group support for the benefit of realizing mutual aid from one's peers, or health education for the prevention and/or maintenance of healthy lifestyles. All of these support services are delivered under the auspices of a qualified, state licensed social worker utilizing a social work service delivery model.
The Division of Substance Abuse Treatment
Support is available for adults, wanting to get clean and sober, through our outpatient treatment program, which is licensed through the Illinois Department of Health Services' Office of Alcohol and Substance Abuse. Priority populations include pregnant women, injecting drug abusers, people with HIV/AIDS, TANF recipients, and DCFS and Department of Corrections referrals.
The Division of Family Support Services
The Division of Family Support Services offers coordinated intensive and comprehensive case management and support services to high-risk pregnant and parenting mothers, infants, and children up to age two. These services are offered through two Illinois Department of Human Services (IDHS) programs; Healthy Start and Healthy Families. The services include the following: case coordination, newborn follow-up, home visitation to participants, interconceptional care, family planning education, health education, maternal depression screening and treatment, parenting classes, and referrals to other community services.
The Division of Youth Intiative
This Division provides health education to area youth within the public school setting. This education curriculum seeks to prevent early pregnancy, violence, STD and HIV infection, and school drop-out rates throught the promotion of character and self-esteem building activities.

Women, Infants and Children Program (WIC)
WIC's goal is to improve the health of low-income pregnant, breastfeeding and non-breastfeeding postpartum women, infants, and children up to five years of age. WIC provides supplemental foods, nutrition education and access to health services. Participants receive vouchers that can be redeemed at retail food stores for specific foods that are rich sources of the nutrients frequently lacking in the diet of low-income mothers and children.
Cook and Shop Smart for Better Health
Cook and Shop Smart for Better Health is a series of health promotion and disease prevention classes offered free of charge and open to community residents. The group classes offer basic nutrition information in a realistic setting where clients can learn shopping skills and prepare low-cost meals. Information is geared toward healthy choices in order to help prevent cancer, heart disease, diabetes, and other chronic diseases.
Operation Frontline
Share Our Strength's Operation Frontline is a national nutrition education program developed to address food security and hunger. Operation Frontline, sponsored nationally by Tyson Foods, Inc., promotes short and long term solutions to hunger by mobilizing volunteer chefs, nutritionists, and financial planners to teach people the nutrition, cooking, food budgeting, sanitation, and financial skills they need to make healthy food choices and good financial decisions on a low-income budget. Frontline activities include six-week training sessions, free groceries to implement skills learned and a certificate of completion.
Mother and Child Program (MAC)
The MAC Program provides a monthly allotment of USDA commodity foods such as juice, evaporated milk, peanut butter, cereals, rice, canned fruits, vegetables, and meat to low-income women, infants and children up to age six. Participants also receive group nutrition education. This program is administered through Catholic Charities, a partner with NNHSC.
Medical Nutrition Therapy
This free service is offered through a physician referral only. A registered dietician provides individual nutrition counseling and follow-up to NNHSC clients in need of nutritional therapy such as diabetes counseling, hypertension counseling, or weight loss counseling.
Breastfeeding Program
NNHSC's Breastfeeding Program recently received a "Loving Support Award" from the Illinois Department of Human Services for its outstanding contribution and innovative ways of increasing the incidence and duration of breastfeeding. A breastfeeding coordinator oversees breastfeeding peer counselors who educate and support women who make the decision to breastfeed. Breastfeeding mothers returning to work or school may be eligible to receive free breast pumps and gift incentives from WIC.
Nutrition Education and Training
The Nutrition Education and Training Program offers dietetic, nursing, medical, and public health students and interns important community based learning and research opportunities in a minority neighborhood. This program is geared toward encouraging students to seek employment in public health nutrition.

About Us
Near North Health Service Corporation is a 501(c)(3) non-profit Federally Qualified Health Center (FQHC). It is one of the largest providers of community-based primary care in Chicago. We provide health care, social services and nutrition education to the medically indigent and uninsured residents of the Near North side (Cabrini Green), West Town, Humboldt Park, West Garfield Park, Austin, Kenwood/Oakland, Douglas and Grand Boulevard, and Uptown communities.
Over the years, Near North Health Service Corporation has been called upon to expand in the face of increasingly diverse community needs. Our first clinic was housed within the Olivet Community Center, more than forty years ago. However, the unique approach to health care that we began back then has proven itself to be a strong foundation that has withstood the test of time.
Our philosophy reflects an approach that anticipates community needs and plans for expansions that are developed in accordance to these needs. Throughout our continuous evolution, we are happy to report that not only have our methods held up under the intense new demands of ever-changing communities, but also we have come through with shining results. We are proud to report that our successes now serve as a national model for excellence in community-oriented primary health care.
Near North Health Service Corporation (NNHSC) targets a primary service area that is spread across many of Chicago's 77 community areas, encompassing a population of more than 1,500,000. A full array of primary care clinical services and comprehensive social support programs are provided through six Near North community-based health centers. While disparate, NNHSC communities are characterized by large concentrations of CHA high rise apartments, abandoned residential buildings, blighted commercial properties, and an utter absence of basic services and health care resources.
In fiscal year 2008, NNHSC served 47, 506 clinical and non-clinical patients through more than 98,508 visits. Almost half of NNHSC users, 51%, are uninsured; 42% have Medicaid coverage, and 7% are covered through Medicare. Females account for 67% of NNHSC patients and males represent 33%. Seventy percent (70%) of NNHSC users live at or below 100% of the Federal Poverty Index (FPI) and another 25% is between 100% to 200% of FPI.

Near North Health Service Corporation (NNHSC) exists to improve the health and well-being of the people and community it serves. It operates programs and services using community primary care concepts, emphasizing prevention of illness and the promotion of healthy lifestyles. Services provided by NNHSC will focus on the medically underserved, and will be
sensitive to the cultural and linguistic needs of the people it serves. Acknowledging that the environmental, social, and cultural factors influence one's health, its programs will empower individuals to be healthy through education and skill building. We will advocate for safe and healthy communities.
NNHSC will play a role in educating and training medical and health care professionals in order to assure the accomplishment of its objective.

In order to fulfill our mission to improve the health and well-being of the communities we serve, our programs extend beyond the boundaries of basic primary care. To us community health means reaching out to people who, for various reasons, have not been drawn into the traditional health care system, such as seniors and men. It means focusing on the prevention of illness and disease through innovative programs that promote good nutrition and other healthy lifestyles. It means developing partnerships with other community
institutions, because by pooling our resources, we can more effectively accomplish the formidable task of expanding access to health care in a time of rising costs and great uncertainty about the status of our nation's health care system. We augment our primary care with social services, health promotion and disease prevention programs that enhance the value of our clinical work. From its inception, NNHSC has maintained a public health perspective that seeks not to only treat illnesses, but identify the root causes of major health problems and develop community-based prevention-oriented programs to address them.
We define health broadly to incorporate issues of environment, lifestyle, and behavioral factors that affect the physical well-being of our communities. These interrelated preventative and curative elements are fundamental to the comprehensive character of NNHSC's approach to health care delivery. This means that providing nutrition education to pregant women, parenting support to first time moms, and social support for at-risk teens are essential components of the clinical work of delivering babies and immunizing children.

Each time you visit one of NNHSC's health centers a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment and a plan for future care or management. This information often referred to as your health or medical record is a:

* Means of communication among the many health professionals who contribute to your care.
* Legal document describing the care you receive,
* Means by which you or a third-party can verify that services billed were actually provided.
* A tool in educating health professionals,
* A source of data for medical research,
* A source of information for public health officials charged with improving the health of this state and nation,
* A source of data for our planning and marketing, and
* A tool with which we can assess and work to improve the care rendered and outcomes achieved.
Understanding what is in your record and how your health information is used helps you to ensure its accuracy; better understand who, what, when, where and why others may access your health information; and, make more informed decisions when authorizing disclosure to others.
Your Health Information Rights
Although your health record is the physical property of NNHSC, the information contained in the record belongs to you. You have the right to:
* Obtain a paper copy of this notice of information practices upon request,
* Inspect and copy your health record,
* Amend your health record,
* Obtain an accounting of disclosures of your health information,
* Request communications of your health information by alternative means or at alternative locations,
* Request a restriction on certain uses and disclosures of your information, and
* Revoke your authorization to use or disclose health information except to the extent that action has already been taken.
Our Responsibilities
Near North Health Service is required to:
* Maintain the privacy of your health information,
* Provide you with this notice as to your legal duties and privacy practices with respect to information we collect and maintain about you,
* Abide by the terms of this notice,
* Notify you if we are unable to agree to a requested restriction, and
