About Us
Near North Health Service Corporation is a 501(c)(3) non-profit Federally Qualified Health Center (FQHC). It is one of the largest providers of community-based primary care in Chicago. We provide health care, social services and nutrition education to the medically indigent and uninsured residents of the Near North side (Cabrini Green), West Town, Humboldt Park, West Garfield Park, Austin, Kenwood/Oakland, Douglas and Grand Boulevard, and Uptown communities.
Over the years, Near North Health Service Corporation has been called upon to expand in the face of increasingly diverse community needs. Our first clinic was housed within the Olivet Community Center, more than forty years ago. However, the unique approach to health care that we began back then has proven itself to be a strong foundation that has withstood the test of time.
Our philosophy reflects an approach that anticipates community needs and plans for expansions that are developed in accordance to these needs. Throughout our continuous evolution, we are happy to report that not only have our methods held up under the intense new demands of ever-changing communities, but also we have come through with shining results. We are proud to report that our successes now serve as a national model for excellence in community-oriented primary health care.
Near North Health Service Corporation (NNHSC) targets a primary service area that is spread across many of Chicago's 77 community areas, encompassing a population of more than 1,500,000. A full array of primary care clinical services and comprehensive social support programs are provided through six Near North community-based health centers. While disparate, NNHSC communities are characterized by large concentrations of CHA high rise apartments, abandoned residential buildings, blighted commercial properties, and an utter absence of basic services and health care resources.
In fiscal year 2008, NNHSC served 47, 506 clinical and non-clinical patients through more than 98,508 visits. Almost half of NNHSC users, 51%, are uninsured; 42% have Medicaid coverage, and 7% are covered through Medicare. Females account for 67% of NNHSC patients and males represent 33%. Seventy percent (70%) of NNHSC users live at or below 100% of the Federal Poverty Index (FPI) and another 25% is between 100% to 200% of FPI.
Mission
Near North Health Service Corporation (NNHSC) exists to improve the health and well-being of the people and community it serves. It operates programs and services using community primary care concepts, emphasizing prevention of illness and the promotion of healthy lifestyles. Services provided by NNHSC will focus on the medically underserved, and will be
sensitive to the cultural and linguistic needs of the people it serves. Acknowledging that the environmental, social, and cultural factors influence one's health, its programs will empower individuals to be healthy through education and skill building. We will advocate for safe and healthy communities.
NNHSC will play a role in educating and training medical and health care professionals in order to assure the accomplishment of its objective.
The composition of the Board of Directors of NNHSC will be representative of the community it serves.

In order to fulfill our mission to improve the health and well-being of the communities we serve, our programs extend beyond the boundaries of basic primary care. To us community health means reaching out to people who, for various reasons, have not been drawn into the traditional health care system, such as seniors and men. It means focusing on the prevention of illness and disease through innovative programs that promote good nutrition and other healthy lifestyles. It means developing partnerships with other community
institutions, because by pooling our resources, we can more effectively accomplish the formidable task of expanding access to health care in a time of rising costs and great uncertainty about the status of our nation's health care system. We augment our primary care with social services, health promotion and disease prevention programs that enhance the value of our clinical work. From its inception, NNHSC has maintained a public health perspective that seeks not to only treat illnesses, but identify the root causes of major health problems and develop community-based prevention-oriented programs to address them.
We define health broadly to incorporate issues of environment, lifestyle, and behavioral factors that affect the physical well-being of our communities. These interrelated preventative and curative elements are fundamental to the comprehensive character of NNHSC's approach to health care delivery. This means that providing nutrition education to pregant women, parenting support to first time moms, and social support for at-risk teens are essential components of the clinical work of delivering babies and immunizing children.

Introduction
Near North Health Service Corporation (NNHSC) is committed to treating and using your protected health information responsibly. This Notice of Health Information Practices describes the personal health information that we collect and how/when we use or disclose this information. It also describes your rights as they relate to your protected health information. This notice is effective April 14, 2003 and applies to all protected health information as defined by federal regulations.
Understanding Your Health Record/Information
Each time you visit one of NNHSC's health centers a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment and a plan for future care or management. This information often referred to as your health or medical record is a:
* Basis for planning your care and treatment,
* Means of communication among the many health professionals who contribute to your care.
* Legal document describing the care you receive,
* Means by which you or a third-party can verify that services billed were actually provided.
* A tool in educating health professionals,
* A source of data for medical research,
* A source of information for public health officials charged with improving the health of this state and nation,
* A source of data for our planning and marketing, and
* A tool with which we can assess and work to improve the care rendered and outcomes achieved.
Understanding what is in your record and how your health information is used helps you to ensure its accuracy; better understand who, what, when, where and why others may access your health information; and, make more informed decisions when authorizing disclosure to others.
Your Health Information Rights
Although your health record is the physical property of NNHSC, the information contained in the record belongs to you. You have the right to:
* Obtain a paper copy of this notice of information practices upon request,
* Inspect and copy your health record,
* Amend your health record,
* Obtain an accounting of disclosures of your health information,
* Request communications of your health information by alternative means or at alternative locations,
* Request a restriction on certain uses and disclosures of your information, and
* Revoke your authorization to use or disclose health information except to the extent that action has already been taken.
Our Responsibilities
Near North Health Service is required to:
* Maintain the privacy of your health information,
* Provide you with this notice as to your legal duties and privacy practices with respect to information we collect and maintain about you,
* Abide by the terms of this notice,
* Notify you if we are unable to agree to a requested restriction, and
* Accommodate reasonable requests that you may have to communicate health information by alternative means or at alternative locations.
NNHSC reserves the right to change its practices and to make the new provisions effective for all protected health information that it maintains. Should NNHSC's information practices change, a revised notice will be mailed to the address you supplied or, if you agree, it will be sent via email.
We will not use or disclose your health information without your authorization, except as described in this notice. We will also discontinue the use or disclosure of your health information after we have received a written revocation of the authorization according to the procedures included in the authorization.
